integrated into facilities. For example, Doctors’ offices are limited to providing all health care needs for

physical limitations will need referrals to see a specialist such as a behavior therapist and psychologist. An integrated
care system is beneficial for patients with multiple health conditions because they have access to critical needs in one

Collaborative documentation is a patient-centered approach, also known as concurrent documentation, which allows
for the client and professional to come together and go over the progressive notes (assessment, service planning,
client-practitioner interactions, and other documentation) as a team to help assist with developing a beneficial plan to
bring about overall positive well-being (Hirsch, 2012). Collaborative documentation allows for the client to add their
own input into their plans, which gives them the opportunity to be engaged in their progress and plan outcomes.
Integrating a system of collaborative documentation helps the team (client and professionals) provide services in a
central location, which can increase results during the plan implementation process; this can increase the quality of life
of the client, which also improves quality of care. Benefits of collaborative documentation include improved
responsibility from clients and professionals in the pursuit of their plan goals, helps keep the client engaged in
theirtreatment set up, and it also improves how the integrative health care team come together to perform care
coordination.
An administrator can increase motivation by showing how the documentation improves plan goals and outcomes, it
can build trust in the professional relationship which can motivate the client to continue the process, and they can
increase motivation by showing them how much collaborative documentation brings about positive change by allowing
